Abstract

About 60 thousand federal public servers, pertaining to the ex-Inamps and the Health Ministry, were placed, in the 80’s and 90’s, at the disposal of the state and municipal secretariats of health, as part of the structural conditions of the decentralization of management that had been proved necessary in order to give origin to the Unified System of Health (USH). In recent years, a substantive fraction of these servers has been retiring, creating a gap of work force in these institutions. The state and municipal managers of the USH understand that, in these circumstances, the Ministry of Health is responsible for finding a solution for the problem, since the condition of cession of this staff was part of the “pact of decentralization”. The paper describes the origins and the current dimensions of this redistribution of the staff of the Ministry of Health, comparing, in terms of supply and of expenses, the groups that are occupied in sectors of this ministry and the ones incorporated by the state and municipal institutions of the USH. In the same way, it compares the trends of retirement of this group with other servers of the federal institutions. The author understands that, in the current conjuncture of the decentralization, the responsibility of replacement of this staff is not incumbent to the Ministry of Health because this would enter in conflict with the main aim of the decentralization, that is to create sustainable local conditions of management and installment of jobs. The author recommends a temporary Plan of Incentives to confer to the human resources of the USH a commitment with productivity, fulltime work and quality of services. The “decentralized” staff would be contemplated selectively in this Plan, in accordance with a criterion of exercise of functions in relevance for the system, in such a way as to help to delay their retirement.
