Abstract
Os aspectos que devem ser considerados no planejamento dos questionários podem ser divididos entre aqueles que se referem à sua forma, à sua estrutura e ao seu conteúdo. Esse texto faz recomendações para o planejamento da forma do questionário e inclui elementos relacionados à formatação gráfica das questões, à apresentação de instruções para preenchimento e processamento, à composição das folhas e cadernos etc. Essas recomendações destinam-se a levantamentos de pequeno porte, mas algumas podem ser úteis a outros tipos de pesquisa mais sofisticadas. Discute-se a relação entre a forma dos questionários e o processo completo de pergunta, de resposta e de registro que caracteriza uma entrevista, e como a redação orientada à leitura, a tipografia orien-tada ao preenchimento e ao processamento, o fornecimento de instruções no ques-tionário, o controle do registro de respostas, a disposição dos enunciados, as instruções e as alternativas de resposta e mecanismos de filtragem de resposta podem ser úteis. Também é apresentado um modelo geral que pode ser usado como guia.

The planning of a questionnaire has to cover aspects such as its form, structure and content. In this text I make a few recommendations related to the form of the questionnaire, which includes the graphic layout of the questions, instructions for filling and processing, the binding and others. These recommendations target small scale surveys but some of them may be of use even for more sofisticated surveys. I discuss how the form of the questionnaires affects the entire process of making the question and registering the answers of an interview. Moreover I show that concerns about the readability, the proper use of typography, providing instructions, answer alternatives and filtering mechanisms can be useful. I also present a general model of question that can be used as a guide.
