Abstract

Neste artigo contrapõem-se as teorias de justiça que embasaram a construção do modelo de provisão de saúde universal no Brasil. O argumento central é que os critérios de justiça postulados não são atingidos na prática, uma vez que não há como se pensar em justiça sem considerar as restrições impostas pela escassez de recursos econômicos. Essa é a questão fundamental a ser tratada pela justiça. Neste contexto, questiona-se a viabilidade do modelo de provisão de saúde pública, universal e gratuita, proporcionar atenção à saúde com a qualidade e a tecnologia demandadas pela medicina atual. Duas opções se apresentam. A mais confortável é continuar com o atual modelo, mesmo sem tornar factível o princípio da universalização equitativa. A outra é repensar o modelo de atenção pública e gratuita, criando-se critérios de acessibilidade, priorização do atendimento e coparticipação dos custos.

This essay compares the theories of justice that founded the set up of a universal health care supply model with the particular way in which the public health sector provides care for the population in Brazil. The main argument states that none of the criteria for postulated justice is reached in practical terms. In fact, there is no way to think about justice without considering the restrictions imposed by the shortage of economic resources. Within this context, the feasibility of the universal and free public health care model to produce health services according to quality and technology required by nowadays medicine is questioned. Therefore, two options appear. The first one is more comfortable, namely to carry on with the current model, which is a fallacy regarding universality. The other option is to reassess the public and free health care model, creating thereby criteria for access, consumer service priority and co-participation.
